Incident Reporting is a very positive tool for:

2. Affording medical treatment
3. Identifying flawed procedures
4. Improving work environment
5. Increasing productivity
6. Increasing safety awareness
7. Ensuring safer environment
8. Benefiting from medical and financial benefits of the Workmen's Compensation Law
9. Highlighting areas of concern
10. Monitoring occurence of incidents
11. Preventing recurrence of same or similar incidents
12. Bringing forward concerns to related University officials
13. Complying with managerial and accreditation requirements

Everyone is encouraged to report incidents... Reporting incidents is in no way used to place blame or to take punitive actions against any individual.
