Submit Service Request

How to submit a Service Request


Call PPD Help Desk at extension 2015 or use the web enabled Work Request Form for all routine maintenance works such as doors sticking, plumbing problems, broken windows, burn-out light bulbs, heating or air-conditioning needs, carpet stains or repairs to classroom furniture or fixtures and non-routine services such as repairs to department equipment, installing new electrical outlets, painting furniture, installing new phone, moving furniture, special events, cost estimates and general minor remodeling projects that does not require engineering.

General Instructions

  1. Campus department requesting material and/or services from Physical Plant must complete all mandatory (*) labeled fields.
  2. Please make sure that information you enter are accurate information. For any inquiry the Help Desk staff can help you filling the form.
  3. After completing the form, please print all copies to be submitted by clicking the "Print" button located at the bottom of the page. Please note that Approval signatures for the desired action are still required.
  4. Upon receipt of the Service Request by Physical Plant Help Desk, a number will be assigned, and an acknowledgement copy will be returned to the requesting department. This assigned number should be referenced in all inquiries or correspondence.
  5. Physical Plant Department will estimate the work and return the Service Request to the customer for approval. Upon approval of work to be done, Budget approval is needed in able to secure funds.
  6. Physical Plant Department will bill the account on a periodic basis (usually monthly) for labor and materials until all work is completed.
  7. Copy of the Service Request will be sent back for the user after completion of to works for his/her signature in approval.
